Concussion Safety

The Walker Recreation Department is required by state law to provide educational materials on the signs, symptoms and consequences of concussions to each youth athlete and their parent(s) or guardian(s) and obtain a signed statement acknowledging receipt of the information for the organizing entity to keep on record.

This page has been posted to make it easier for you to find the information you need about the parents’ legal requirements for concussion safety.

Instructions for Parents and Players

  1. Read and keep this document: A Fact Sheet for Youth Sports Parents
  2. Read, sign, and return this document: Parent and Athlete Concussion Information Sheet
  3. The player and the parent must both sign the Parent and Athlete Concussion Information Sheet (step #2, above) and return it to the Walker Recreation Department. You need only do this once, as long as the Walker Recreation Department has your child’s sheet on file.
